The forms and instructions at this site are for use by Local Road Agencies,
Consultants, and Engineers wishing to submit Environmental Survey Request Forms
to an IDOT District Office.
The Environment Section of the Bureau of Design and Environment has an Access
database for tracking the Environmental Survey process. This database is called
the Project Monitoring Application (PMA). PMA allows for the electronic
submittal of form information for an Environmental Survey Request (ESR) and
various other forms associated with the environmental clearance process.
SPECIAL TIP: In order to avoid unnecessary difficulties, it is suggested
that form instructions be read before beginning data entry for the first time.
Leaving the form to view the instructions will result in the form being cleared
of all entries. Therefore, it may be beneficial to print the instructions so a
hard copy can be referred to during data entry.
